What does a chronic care assessment involve?
One of our GPs will conduct a thorough assessment of your health needs and work with you to set goals, determine an action plan, and identify what treatments and ongoing services you need to optimise your health and quality of life.
They can also assist you in accessing Medicare benefits and prepare Team Care Arrangements, which help to reduce the cost of allied health visits.
Your GP may recommend medication reviews with your pharmacist and host regular care plan reviews to ensure your care needs are being adequately met.
For patients aged between 45-49 years or aged over 75, there are special Medical rebates available for routine health assessments.
