We’re pleased to announce that we are taking part in the national rollout of the COVID-19 vaccine rollout. Our clinic is currently administering the Pfizer, AstraZeneca and Novavax vaccines.
Patients aged 5 and over are eligible for the Pfizer vaccine.
Patients aged 60 and over are also eligible for the AstraZeneca vaccine.
Patients aged 18 and over are eligible for the Novavax vaccine.
Below you will find information for your reference as well as instructions on how to book and reserve your appointment with us if you are eligible.
Am I eligible for the current phase?
You will need to check your eligibility, you can review that using the Australian Government Department of Health Vaccines Eligibility Checker here.
You may be required to complete an eligibility declaration. The official Covid-19 vaccination eligibility declaration form can be located here.
You will need to consent to receiving your Covid-19 vaccine, you can do this online when you reserve and book your appointment with us, alternatively you can access the consent form here.
Your care and everyone’s safety are our priorities.
Due to high levels of Covid in the community, we kindly request that you DO NOT enter the clinic if you have a sore throat, running nose, fever, cough, diarrhea or even mild flu like symptoms. Instead book a telephone or video appointment through the booking link on our website, the Automed AMS CONNECT smartphone app, or ring reception on 9339 7799. You are encouraged to get a Covid PCR test or if this is not practical, do a RAT test at home prior to your appointment. Please isolate at home until your symptoms resolve or as advised by the doctor during your consult. If a face-to-face consult is deemed necessary by your doctor, it will be done in the surgery carpark using personal protective equipment, for the safety of our staff and other patients.
For all other patients, clinic consults are still available where there is a clinical need to be seen face to face, so long as you have not been diagnosed with Covid, have no Covid type symptoms and are not a close contact. To minimise the number of people in the clinic at any one time, you may be asked to wait in your car or outside the clinic before being contacted on your mobile phone to enter. Please follow instructions and wear a surgical mask.
