We’re pleased to announce that we are taking part in the national rollout of the COVID-19 vaccine rollout. Our clinic is currently administering the Pfizer and AstraZeneca vaccines.
Patients aged 12 and over are eligible for the Pfzier vaccine.
Patients aged 60 and over are also eligible for the AstraZeneca vaccine.
Below you will find information for your reference as well as instructions on how to book and reserve your appointment with us if you are eligible.
Am I eligible for the current phase?
You will need to check your eligibility, you can review that using the Australian Government Department of Health Vaccines Eligibility Checker here.
You may be required to complete an eligibility declaration. The official Covid-19 vaccination eligibility declaration form can be located here.
You will need to consent to receiving your Covid-19 vaccine, you can do this online when you reserve and book your appointment with us, alternatively you can access the consent form here.

To protect the health of our staff and other patients, we have a strict COVID-19 policy at our clinic:
- If you are experiencing flu or cold-like symptoms including a fever, cough, or sore throat, please ring us instead to book a phone consultation.
- If you are experiencing a fever or have difficulty breathing, have been in contact with someone with a positive COVID-19 result, are a healthcare worker, or are not eligible for a phone consultation with your GP, please go to the COVID Clinic at Fiona Stanley Hospital to get tested immediately.
We strongly recommend booking a consultation with your GP if you have any concerns or questions about the vaccine.
